- Photos of the outside of the box and the inside with the packaging. We need to view all sides of the box, the tape, and shipping labels to help determine what may have happened.

Once we receive all the information requested, we can then proceed with the claim. But please act on this soon as you only have 30 days since the date of shipment to file a claim. We must receive the completed paperwork within 30 days of shipment in order to be considered for claim.
