当社は、JISQ15001:2006 の要求事項に則り、Ⅱの１から11に定めた個人情報を「開示対象個人情報」とし、当該開示対象個人情報については、本人又はその代理人に限り、利用目的の通知・開示・訂正・追加・削除、及び利用の停止又は消去、第三者への提供の停止を請求することができますので、下記までお申し出下さい。
その際、個人情報の漏洩や情報の書き換え等を防止するため、本人確認が可能な書類・資料をご提示していただき、当社の定めた手順にて対応させていただきます。
