Regarding the incident of motion for sender information discovery provisional disposition order made out by the above creditor, it is decided to hear your claim.
In line with this, you are requested to come to the issue section of our department (north side on the second-floor) on O, Oth.
申立書および既に裁判所に提出された証拠書類は、債権者（代理人）からあなたに直接郵送されます。
来庁された際には、この通知書を発令係に示してください。また、ご本人であることを確認できる運転免許証、健康保険証等の身分証明書を当日必ず持参してください。
なお、当日来庁されない場合には、あなたの主張（言い分）を聞かないままで仮処分発令が発せられることもありますから、念のため申し添えます。
The petition and the documentary evidence already submitted to the court will be directly mailed to you by the claimant (or their representative).
When coming to the prefectural office, please present your notice/summons to the one in-charge of announcements / bailiff. In addition, please make sure to bring proof of identification such as your driver's license or health insurance card etc. to confirm your identity.
Please take note that if you do not come to court on that day, a temporary injunction may be issued without hearing your statement (objections).
