先週送っていただいた書類ですが、私はどう記入したらいいのかわかりません。

フォワーダーに確認したところ、現地代理店が記入するものではないか？とのことでした。
今回、FOBで発送とのことですが、現地代理店は御社指定の通関業者を使われるのでしょうか？
もし御社指定の通関業者があれば、そちらにBLの作成を依頼してください。

もしこちら指定の通関業者に委託するということでしたら、連絡先は下記になります。
こちらにすみませんが、先週いただいた書類を転送して記入してもらってください。

In regard to the document that you sent us last week, I don't know how to fill out forms.

I confirmed this with my forwarder that the document might be filled out by local agent.
This time, the document will be delivered via FOB.
Will the local agent use the custom broker that you specifies for the delivery?

If you do have the customer broker that you specifies, please request the broker to create BL.
