•外国人登録証明書（みなし特別永住者証明書）
•3箇月以内に撮影した写真1枚（横3cm×縦4cm，無帽で正面を向き，背景がないもの）
•窓口に来られる方の本人確認書類（特別永住者証明書，外国人登録証明書（みなし特別永住者証明書），健康保険証など）

　※本人または同一世帯の親族及び同一世帯の法定代理人以外の方が申請される場合は，事前に必要書類についてお問い合わせください。

- Valid passport (not required for those who don't have passport)
- Alien registration certificate (deemed resident card)
- Photo taken within the last 3 months (3cm wide x 4cm height, taken in full-face view directly facing the camera not wearing a hat, taken in front of a plain background)
- Identification of the applicant (Special permanent resident certificate, Alien registration certificate (deemed special permanent resident certificate), Health insurance card etc)
