Thank you for your recent order with Urban Outfitters. We require additional information to complete your order. We have been unable to verify your name and address with your credit card company.

In order to successfully process your order, we need you to provide, via fax or email, a copy of your latest credit card statement showing the cardholders name, address, and at least the last 4 digits of the card (all other sensitive information may be blocked out) and a photo ID, i.e. drivers license, state identification or passport.

If you cannot provide the information above, a second option is available.
