前から話している通り問題なのは誰がミスを犯したかではなく、
なぜまたミスが起こったのかが問題です。

2重、3重のチェックを行なっていると思っていたので、
今回のミスによる損失は私にとって想定外です。

現在の価格が本当に正しいものなのか、
再度チェックをするよう依頼して欲しい。

スピードは早いけれども1回のミスで信じられないような損失を被ります。

過去のプロジェクトにかかった費用は、度重なるミスによる損失で、
正直まったく採算がとれていません。

昨日はなぜ連絡無しに休んだのですか？

The point of the story is not who has made the mistake, but why something went wrong again.
