本症例は2007年に認められなかった動脈解離が2009年に出現し、2012年に自然消退した。それとは別に違う箇所に新たに出現し、血管内超音波でその内腔を観察できた。新たに動脈解離が出現した領域の内腔像は瘤内に血栓の付着を伴う潰瘍性病変が認められ、局所的に低輝度で繊維性被膜との境界が不明瞭な脂質に富んだプラークが認められた。wireおよびIVUS操作が原因で同部位に容易に動脈解離を来したことから、動脈解離病変は脆弱な血管壁を有することが考えられる。

In 2009 the artery dissection that went unwritten in the original case in 2007 appeared, and in 2012 it spontaneously regressed. It then appeared in an entirely different location and could be observed in that lumen through IVUS. In the lumen of the newly-appeared artery dissection spot, blood clot adhesion accompanied with ulcerous lesions inside bumps were observed. Plaque rich in indistinct lipids that had a fibrous membrane border were observed locally at low brightness. Since artery dissection was easily induced by wire and IVUS operations in the same locations, it is possible that the pathological change of artery dissection is having a frail vascular wall.
