現在タウンページが手元にないため、インターネット上のタウンページに掲載されている部分を印刷したものを送っています。

I faxed my dealer application.
As for a copy from the yellow page, it is from the town page which is just like the yellow page in Japan.
Because I currently don't have a physical copy of the town page, I printed out the page from online.
