ホーム画面に存在するアイコン数が、登録できる上限数に達していると、
"Add To Home Screen"の項目は表示されません。
もしこの状況であれば、
不要なアイコンを削除するか、フォルダを作って複数のアイコンをひとつに
まとめるなどしてホーム画面に空きを作れば、新しいアイコンを
作成できるはずです。

もしこのケースと違うのであればその状況を詳しく教えて下さい。
当方のiPad2では問題なく動作しています。
