以上のとおり出席があったので、本総会は適法に成立した。
定刻、定款の規定に基づき　代表取締役xx　議長席につき、開会を宣し、直ちに議事に入った。
第1号議案　目的変更に関する件
議長は、当会社の目的を変更したい旨を述べ、その理由を詳細に説明した。これに伴い、当会社の定款を下記のとおり変更したい旨を議場に諮ったところ、満場一致をもってこれを承認可決した。
