Dr. Arnaout, can you tell us the
clinical impressions that the team caring for this
patient had?
Dr. Arnaout: We thought that his abdominal pain
at presentation was due to the renal infarcts,
caused by arterial emboli from the thrombus associated
with the left ventricular aneurysm. An
ischemic infarct due to coronary artery disease
seemed to be ruled out by the absence of coronary
atherosclerosis. We considered Chagas’ disease
because of the combination of cardiac and gastrointestinal
symptoms, although the absence of
global ventricular dilatation was unusual. Serum
was sent to the Centers for Disease Control and
Prevention for testing for Chagas’ disease.

アルナウト氏：私達は発表の中で、彼の腹部の疼痛は左心室の動脈瘤に伴う血栓による動脈梗塞が原因の腎梗塞によるものだと考えました。
虚血性梗塞の原因は冠動脈粥状硬化症を除く冠動脈疾患だと思われます。私達はシャガス病だと考えました。それは、心室以外では稀な肥大症が胃の噴門部と胃腸の症状として見られた為です。シャガス病の検査の為、血清は疾病管理予防センターへ送られました。
