私清澤満希郎は、以下の件について承諾いたします。
私は血液透析に大きな疑念を持っており、リスクがあっても何が何でも腎臓移植したいと考えております。
医師から指摘された3点についてすべて承諾いたします。
1、がん治療後5年に満たない事。
実質4年目ですが、
2、基準年齢65歳を超える69歳であること。
3、動脈硬化におけるリスクがあること。
以上3点において、日本を含む国際基準においてもハイリスクであることを理解した上で手術に臨みたいと思います。
I, 清澤満希郎, accept about the following matters.
I have a great deal of skepticism toward haemodialysis and want to transplant kidney at any cost even if it has a risk.
I accept all of three points that the doctor pointed out.
1, Less than five years aftter cancer therap.
This is the fourth year substantially,
2, Being 69 years old beyond the criteria age, 65 years old.
3, At the risk of arterial sclerosis.
In above three points, I want to undergo the operation after having understood that it is high-risk in the international standard including Japan.

以上、手術におけるリスクを踏まえた上でここに手術を希望いたします。
万が一の事態が起こった場合でも一切の責任について病院等に責任を寄せることは致しません。
(In Japan, it is judged to be a high-risk matter, but operable.)
I hope for the operation here with the risk in the operation in mind.
I will never put any responsibility to the hospitals if the worst accident happens.
