We don't have any form for that. I think it's quite simple with the title is notification about changing name and address. The receiver will be us, and the contents is the old name, address will change into new name and address, signature and stamp. You should do that. Scan and send me and Mr Gohn also. And if possible send the documens with the documet of export. You can not change anything in the document now so please send those document for us as soon as possible.
