本症例では、1回目の手術後に低体温の合併症が起きた。鎮静による過度な副交感神経活動の亢進が原因の一つと考えた。しかし、2度目の鎮静後には体温低下がなかったため、DEXの影響による可能性が強く疑われた。鎮静中の血圧は1回目の鎮静の方が2回目の鎮静の時より約15％低くなっていた。これからも1回目の鎮静時の方がより一層の副交感神経活動の亢進があった可能性が考えられる。
DEX、ミダゾラム、プロポフォールの血中半減期は、それぞれ2.5h、1.8-6.4h、30-60minである。

With this case, a complication of hypothermia arouse after the first operation. It is thought that the accentuation of excessive parasympathetic nerve activity from the sedation is one of the causes. However, no hypothermia arouse after the second operation, so the possibility of DEX effect was strongly suspected. The blood pressure during the first sedation was lower than the second one by 15%. From this fact,it is considered that there was possibility of more accentuation of parasympathetic nerve activity during the first sedation.
Half-life in blood of DEX, midazolam and propofol are 2.5h,1.8-6.4h and 30-60min respectively.
