I have visited the post office for what must be the last time on this issue!

Attached and appended below are the forms the U S Post Office filled out and faxed - Both of which I have done.
The post office representative also put in her name and the office address on the form as required.

Note: They inspected the shipping box, and the broken cover, and then they KEPT ALL of the parts (Box, cover, broken pieces, etc.).

I hope you can do the rest on your own as I am now finished with this project.

以下に添付されたのは米国の郵便局が記入及びファックスしたフォームであり、両方とも私が担当しました。
郵便局の代表者も必要なフォームへ自分の氏名及び同局の所在地を記入しました。

注意：同局の職員が出荷する箱及び破損したカバーを検査後、全体（箱、カバー及び破損した部分等）を保管しました。

これで私は本件から手を引きますので、残りは貴方ががんばってください。

"representative"が｢代表者｣よりも｢窓口担当者｣のほうがしっくりくると思いますが、全体の訳は分かりやすいと思います。
